摘　　要：目的:对比分析不同心电图诊断标准对高血压致左心室肥厚(LVH)的诊断价值。方法:回顾性分析2018年7月1日—2018年12月31日就诊的原发性高血压患者150例,评价心电图Sokolow-lyon电压标准、Cornell电压标准、Peguero-Lo Presti标准和超声心动图诊断结果的一致性,以超声心动图作为诊断LVH的参考标准,评估心电图各标准敏感度及特异度。结果:心电图对LVH的检出率低于超声心动图,两者比较,差异具有统计学意义(P<0.05);不同心电图标准与超声心动图比较,Kappa值分别为0.299,0.307,0.628,其诊断LVH敏感度分别为29.4%,31.4%,60.8%,特异度分别为96.0%,94.9%,97.0%。结论:心电图对LVH的检出率不及超声心动图;Peguero-Lo presti标准敏感度较高,与超声心动图的检查结果一致性好,且简便易行,应用前景良好,但尚需在不同人群大样本中进一步验证。Objective:To comparatively analyze diagnostic efficacy of different ECG diagnostic criteria for left ventricular hypertrophy caused by hypertensionin.Methods:A retrospective analysis was made of 150 patients with primary hypertension who were treated in the Department of Cardiovascular Medicine,Affiliated Hospital of North Sichuan Medical College from July 1 to December 31.To evaluate the consistency of Sokolow-lyon voltage standard,Cornell voltage standard,Peguero-Lo Presti standard and echocardiographic diagnostic results.Echocardiography was used as the reference standard for the diagnosis of LVH to evaluate the sensitivity and specificity of ECG criteria.Results:There were statistical differences of detection rate between ECG and Echocardiography(P<0.05),compared with echocardiography,the Kappa values were 0.299,0.307 and 0.628,the sensitivity,specificity of diagnosing LVH were 29.4%,31.4%,60.8%and 96.0%,94.9%and 97.0%respectively.Conclusion:The detection rate of electrocardiogram for LVH is lower than that of echocardiography;SD+SV4 standard has higher sensitivity,it has good consistency with echocardiographic results and it is simple and feasible.It has a good application prospect,but it needs to be further validated in large samples of different populations.
